OFFICIAL RULES

 

1.General.  NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING.  By entering The Ultimate Leap Sweepstakes (the “Sweepstakes”), co-presented by Roundy’s Supermarkets, Inc. and Robert Haack Diamonds and promoted by the Milwaukee Journal Sentinel (the “Promoter”) and the Green Bay Packers (collectively, the “Presenters”), the entrant acknowledges and agrees to all of these Official Rules.  Void where prohibited.  Sweepstakes begins on September 2, 2013 at 12:00am Central Time (“CT”), and ends on November 24, 2013 at 11:59pm CT (the “Entry Period”).  The Sweepstakes will include a bonus entry period for an opportunity to win a bonus prize, which shall run from November 25, 2013 at 12:00am CT to December 1, 2013 at 11:59pm CT (“Bonus Entry Period”).  By entering, entrants agree to waive any right to claim any ambiguity or error in these Official Rules, or the drawing itself, and agree to be bound by these Official Rules and by all decisions of the Presenters, whose decisions are binding and final.  Failure to comply with these rules may result in disqualification from this sweepstakes.

 

2.Eligibility.  The Sweepstakes is open to all legal residents of the United States and the District of Columbia who are 18 years of age or older at the time of entry. Employees of Milwaukee Journal Sentinel, the Green Bay Packers, Roundy’s Supermarkets, Inc., and Robert Haack Diamonds, and each of their respective parent, subsidiary, and affiliated entities (collectively, the “Promotion Entities”), the Promoter’s advertising agencies, participating sponsors, and promotional partners, and the members of their immediate families (spouse, parents, siblings or children) or households are ineligible to participate or win.  This Promotion is subject to all applicable federal, state, and local laws and regulations.  Void outside the State of Wisconsin and where prohibited.  Only one winner per household is permitted in any Promoter-conducted contest.  Entrants are required to provide truthful information and Promoter will reject and delete any entry that it discovers to be false or fraudulent.  Promoter will disqualify any entry from individuals who do not meet the eligibility requirements.

 

3.How to Enter.  To participate in the Sweepstakes, you may enter via the following method(s) during the Entry Period.  To enter for a chance to win only the bonus prize, described below, you may enter via the following methods(s) during the Bonus Entry Period.  There is no limit to the number of entries regardless of the method of entry during either entry period.

 

INTERNET – Visit http://www.jsonline.com/theleap and follow the instructions to enter and submit your first name and last name, complete address, city, state, zip code, telephone number and a valid email address in the online entry form.  Internet entries will be deemed made by the authorized account holder of the email address submitted at the time of entry.  The authorized account holder is the natural person who is assigned to the email address by an internet access provider, online service provider or other organization that is responsible by assigning the email address or the domain associated with the submitted email address.  Multiple entrants are not permitted to share the same email address.  All entries submitted become the sole property of the Promoter and will not be acknowledged or returned.  Use of any device to automate entry is prohibited.  Proof of submission of an entry shall not be deemed proof of receipt by Promoter.  The contest Promoter’s computer is the official time keeping device for the Sweepstakes.  For purpose of this Sweepstakes, a “Day” shall begin at 12:00:00am CT and shall end at 11:59:59pm CT.

 

IN PERSON (Roundy’s) – Roundy’s Consumers who purchase the weekly pre-designated item at participating Roundy’s owned and operated Wisconsin Pick ‘n Save, Copps, and Metro Market locations and present their Roundy’s “Fresh Perks” card at time of purchase will receive one (1) entry with such qualifying purchase.  The weekly pre-designated item will be announced each week in the weekly print and digital promotional advertisements.  One (1) entry will be submitted to the registered Roundy’s “Fresh Perks” cardholder for each qualifying purchase when the Roundy’s “Fresh Perks” card is presented at time of purchase, regardless of the number of qualifying items purchased in each transaction.  It is solely the responsibility of the Roundy’s “Fresh Perks” cardholder to verify that their contact information associated with their Roundy’s “Fresh Perks” card is current and accurate.  To enter at Roundy’s without a purchase, you may obtain an official entry form (available while supplies last) at the participating Roundy’s store customer service desk.  Legibly hand write your entry information on an official entry form and place the completed entry form in the registration box.  All entries must be received during the Entry Period, or, for a chance to win the bonus prize, during the Bonus Entry Period.  Entries submitted may not be acknowledged or returned.  Failure to provide all required information will result in disqualification.

 

IN PERSON (Robert Haack Diamonds) – Visit participating Robert Haack Diamonds locations during the Sweepstakes dates to obtain an official entry form (available while supplies last).  Legibly hand write your entry information on an official entry form and place the completed entry form in the registration box.  All entries must be received during the Entry Period, or, for a chance to win the bonus prize, during the Bonus Entry Period.  Entries submitted may not be acknowledged or returned.  Failure to provide all required information will result in disqualification.  Participating Robert Haack Diamonds locations:  (i) 7530 West Layton Avenue in Milwaukee and (ii) 6969 North Port Washington Road in Glendale.

 

4.Prizes.  Twenty-five (25) Third Place Prizes:  a $50 Robert Haack Diamonds gift card and a $50 Packers Pro Shop gift card.  Third Place Prize Approximate Retail Value (“ARV”):  $100 each.

 

Ten (10) Finalist Prizes:  Each Finalist will receive instructional training on or around December 3, 2013 in a classroom setting by a Packers player at Lambeau Field.  Each Finalist will be required to sign a participation release prior to participating in the training.  In addition, each Finalist will be given an opportunity to attend an “Ultimate Leap” tryout on a date pre-determined by the Packers.

 

Nine (9) Second Place Prizes:  a jersey autographed by the Packers player providing the “Ultimate Leap” training.  (Second Place Prize ARV:  $100 each.)

 

One (1) Grand Prize:  An opportunity to take the “Ultimate Leap” at Lambeau Field on December 22, 2013 during the Packers game at Lambeau Field against Pittsburgh.  The “Ultimate Leap” is leap to the top of the padded wall behind the end zone at Lambeau Field.  The Grand Prize winner will also receive the game ball from the December 22, 2013 game; an authentic Green Bay Packers team jersey with his/her name on the back; four (4) VIP sideline passes for pre-game; four (4) Club Seats to the December 22, 2013 game, and one-night hotel accommodations in Green Bay, Wisconsin for four persons (winner and three guests).  Grand Prize ARV:  $2,182.

 

One (1) Bonus Prize:  A trip for two (winner and guest) to a 2014 Green Bay Packers Away Game on a game day to be selected solely by the Milwaukee Journal Sentinel.  Bonus Prize shall include roundtrip transportation for two, overnight accommodations for two (one night, standard, double occupancy), and two game day tickets.  Bonus Prize trip must be completed by the end of the 2014 Green Bay Packers Regular Season.  Bonus Prize ARV:  $1,980.  Actual Bonus Prize ARV may vary depending upon airfare and lodging rates in effect at time of travel.  Certain travel restrictions may apply.  Travel arrangements must be made through the Promoter or its designee.  Prize is subject to availability.  If the winner cannot travel on date(s) specified by the Promoter the winner may be disqualified, and an alternate winner may be selected (time permitting) at the Promoter’s sole discretion.  All other costs not specifically stated herein, including but not limited to those of, taxes, meals, tips and ground transportation to/from the airport and/or to/from the hotel, are the responsibility of the winner.  The winner and guest(s) must travel on the same itinerary and are responsible for obtaining all required travel documents, including, without limitation, valid government-issued photo identification.  Airline regulations apply.  The winner may be required to present a valid credit card upon hotel check-in to cover any incidental expenses incurred during the winner’s stay.  The winner and guest(s) must sign a Publicity, Liability Waiver and Travel Release form prior to traveling.  If a prize-related event or travel is unable to take place as scheduled, for reasons such as cancellation, preemption, postponement or unavailability, including for weather, or for any reason beyond the control of the Promotion Entities, their sole responsibility to the winner will be to award the remaining available elements of the prize and no substitution or compensation will be provided for the unawarded elements of the prize.

 

All other costs not specifically stated herein, including but not limited to those of, taxes, meals, tips and ground transportation to/from Green Bay are the responsibility of the winner.  The winner may be required to present a valid credit card upon hotel check-in to cover any incidental expenses incurred during the winner’s stay.  The winner and guest(s) must sign a Publicity, Liability Waiver and Travel Release form prior to traveling.  If a prize-related event or travel is unable to take place as scheduled, for reasons such as cancellation, preemption, postponement or unavailability, including for weather, or for any reason beyond the control of the Promotion Entities, their sole responsibility to the winner will be to award the remaining available elements of the prize and no substitution or compensation will be provided for the unawarded elements of the prize.

 

Total ARV for all Prizes:  $7,562.

 

There is no substitution, transfer, or cash equivalent for prizes, except that Promoter may, at its sole discretion, substitute prizes of comparable value or cash.  The prizes are expressly limited to the item(s) listed above and do not include taxes, gratuities or any other expenses.  Promoter is not responsible for winners' limitations that prevent acceptance, use, or nonuse of prize. Entrants acknowledge that the Released Parties, as defined herein, have not made, nor are in any manner responsible or liable for, any warranties, representations or guarantees, and hereby disclaim any and all warranties expressed or implied, in fact or in law, whether now or hereafter enacted concerning any prize, including without limitation, implied warranties of quality, merchantability, mechanical condition, or fitness for a particular purpose, except for any express manufacturer’s warranty as may be included with the prize or as may be separately provided to participants by law.  Other restrictions may apply.

 

Any gift cards are subject to the terms and conditions as set forth by the issuer of the gift card.  Such gift cards are not refundable or transferable, and may not be substituted or exchanged for cash or credit at any time, nor will such certificates or coupons be replaced if unused, lost, stolen, or damaged.

 

5.Selection and Notification of Winners.  Decisions of Promoter management with respect to the Sweepstakes are final, including winner selection.  Thirty-five (35) Semi-Finalists will be selected by the Promoter in a random drawing of all valid entries received.  Semi-Finalists need not be present to win.  The random drawing will be held between November 25, 2013 and November 29, 2013.  Odds of being selected as a Semi-Finalist depend on the number of entries received.  Semi-Finalists will be notified by the Packers by phone and/or email immediately following the drawing.

 

From these thirty-five (35) Semi-Finalists, ten (10) will be selected by the Green Bay Packers as Finalists in a random drawing of all Semi-Finalists between November 25, 2013 and November 29, 2013.  Odds:  2:7.  Each of the ten (10) Finalists will be given an opportunity to attempt an “Ultimate Leap” tryout on a date pre-determined by the Packers (Currently scheduled for December 3, 2013).  One (1) Finalist will be selected by the Green Bay Packers designee to take the “Ultimate Leap” on December 22, 2013 during the Packers game at Lambeau Field against Pittsburgh based on Green Bay Packers pre-determined criteria.

 

In addition, one (1) potential Bonus Prize winner will be selected by the Promoter in a random drawing of all valid entries received during the Bonus Entry Period.  Only eligible entrants who enter during the Bonus Entry Period shall be included in the drawing for the Bonus Prize.  Bonus Prize drawing shall be held on December 11, 2013.  Odds of winning depend on the number of entries received during the Bonus Entry Period.

 

Winners will be notified by phone or e-mail.  Winners must execute and return any required affidavit of eligibility and/or liability/publicity release within ten (10) days of notification attempt or winner will be disqualified and the prize will be forfeited and an alternate winner may be chosen by random drawing (time permitting).  If a potential winner cannot be contacted or fails to complete, sign and return the required affidavit of eligibility and liability/publicity release within the required time period, or if a prize or prize notification is returned as undeliverable, potential winner will be disqualified and will forfeit the prize.  Unless otherwise specified in the prize notification, third-place prizes may be picked up at the office of the Promoter at the address below.  The winner will forfeit any prize or prize certificate not claimed within ten (10) days of winning.  Second-place prizes and the Grand Prize will be distributed by the Green Bay Packers.  The Promoter, its sponsor, and/or Promotional Partner are not responsible for the safe arrival of a prize or prize certificate.

 

Guests must be 18 or older, unless accompanied by a parent or legal guardian.

 

6.Conditions.  Payments of all federal, state and local taxes are solely the responsibility of the winners.  Winners will be required to complete and submit an IRS Form W-9 or the equivalent, including the winner’s full Social Security Number for receipt of any prize valued at $50 or more or for any prizes awarded by Promoter.  Failure to submit a completed Form W-9 or equivalent will result in disqualification and forfeiture of the prize.  Participating in the Promotion and acceptance of any prize constitutes each entrant’s and winner’s (and guest’s where applicable) permission for the Promoter or its agents to photograph, film, record that entrant, winner ,or guest and to use his or her name, city, state, voice, or likeness, and any statements made by him or her regarding the Promotion or its sponsors for purposes of trade, publicity or promotion without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.  Prior to awarding any prize or prize certificate, and depending on the nature of the prize and eligibility requirements of the contest, the Promoter in its sole discretion will require verification of Promotion winner’s identification by a showing of valid government-issued photo identification.  By entering, participating and/or accepting a prize in the Promotion, each entrant, winner, and guest if applicable, agrees not to sue and to release, discharge, indemnify, hold harmless, and absolve the Promotion Entities, their sponsors, presenters, and promotional partners, their advertising and promotion agencies, and each of their respective parent, subsidiary and affiliated entities and the officers, shareholders, directors, employees, agents, and representatives and the successors and assigns of each of them (the “Released Parties”) from and against any and all claims, demands, actions, suits, losses, expenses, costs, damages, and liabilities including, without limitation, interest, penalties, court costs and reasonable attorneys’ fees and expenses arising directly or indirectly from the prize or participation in the Promotion, including, but not limited to, personal injury, death, or damage to or loss of property, which may occur in connection with, preparation for, travel to, or participation in Promotion, or possession, acceptance and/or use or misuse of prize or participation in any Promotion-related activity, including but not limited to any claims based on publicity rights, defamation, or invasion of privacy and merchandise delivery.  Promoter may require eligible Promotion winners and their guests or travel companions, if any (as well as each of their parent(s) or legal guardian(s), if winners, guests or travel companions are under the age of majority in their state of residence), to sign a liability release confirming such consent.  Except where prohibited, by entering the Promotion, each entrant agrees that: (1) any and all disputes, claims and causes of action arising out of or connected with this Promotion or any Prize (s) shall be resolved individually, without resort to any form of class action, and exclusively by state or federal courts situated in Milwaukee, WI; (2) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, but in no event shall such costs include attorneys' fees; and (3) no punitive, incidental, special, consequential or other damages, including, without limitation, lost profits may be awarded (collectively, "Special Damages").  Further, by entering the Promotion, each entrant waives all rights to claim Special Damages and all rights to have such damages multiplied or increased.  Promoter, in its sole discretion, reserves the right to disqualify any person (and all of their entries) from this Sweepstakes if he or she tampers with the entry process, the operation of the Sweepstakes, or the operation of Promoter’s web channel or is otherwise in violation of the rules.  Promoter further reserves the right, at its sole discretion, to modify, cancel, terminate or suspend the Sweepstakes, or any part of it, if it is not capable of completion as planned or if any fraud, technical failures or any factor beyond the Promoter’s control, including infection by computer virus, bugs, tampering, unauthorized intervention or technical failures of any sort, corrupt or affect the administration, security, fairness, integrity or proper conduct of the Sweepstakes. Any attempt by an entrant or any person to deliberately damage any Promoter website or to undermine the legitimate operation of this Sweepstakes may be a violation of criminal and civil laws.  Should such an attempt be made, Promoter reserves the right to seek full prosecution and/or damages from any such individual to the fullest extent permitted by law.  Promoter’s failure to enforce any term of these Official Rules shall not constitute a waiver of this provision.  If due to circumstances beyond the control of Promoter, any competition or prize-related event or travel is delayed, rescheduled, postponed or cancelled, Promoter reserves the right, but not the obligation, to modify, suspend or cancel the Sweepstakes and shall not be required to award a substitute prize.  The Released Parties are not responsible for (i) typographical or other errors in the printing, the offering or the administration of the Sweepstakes, or in the announcement of a prize; (ii) entries not received due to difficulty accessing the internet, service outage or delays, computer difficulties, malfunctions, disconnections, other technological failures, telephone service outages, delays, dropped calls, or busy signals; or (iii) lost, stolen, mangled, misdirected, illegible, incomplete, incorrect, or late entries.

 

7.Official Rules.  To obtain a copy of the Official Rules or a list of winners following completion of the Sweepstakes (please specify which), send a self-addressed, stamped envelope specifying “Official Rules” or “Winner List” to the Ultimate Leap Sweepstakes Winner List or Official Rules Request, Milwaukee Journal Sentinel, 333 W. State Street, Milwaukee, WI 53203.  A copy of the Official Rules and a list of winners (when complete) are also available during regular business hours at Milwaukee Journal Sentinel, 333 W. State Street, Milwaukee, WI 53203. All such requests must be received within thirty (30) days following completion of the Sweepstakes.

 

8.Terms of Use Agreement and Privacy Policy.  By entering this Sweepstakes, entrants agree to use of their personal information as described in the Privacy Policy.  By use of the Sweepstakes’ website to enter this Sweepstakes, entrants agree to the Promoter’s Terms of Use Agreement.
