1. Winner must be 21 years old and legal US resident.
2. General. By submitting an entry to this sweepstakes, sponsored by NBC Universal (the "Sponsor") the entrant acknowledges and agrees to all of these Official Rules. Sweepstakes begins on March 24, 2010 12:00 AM and ends on April 21, 2010 at 11:59 PM. All times in this sweepstakes refer to Central Time. By entering the sweepstakes, entrants agree to waive any right to claim any ambiguity or error in these Official Rules, or the sweepstakes drawing itself, and agree to be bound by these Official Rules and by all decisions of the Sponsor, whose decisions are binding and final. Sponsor reserves the right to withdraw the transmission of this sweepstakes and award the prize from among entries received prior to the date of the withdrawal. Failure to comply with these rules may result in disqualification from this sweepstakes.
3. Eligibility. The contest is open only to legal U.S. residents who are 21 years of age or older as of 3/24/2010 and is void wherever prohibited or restricted by applicable federal or state laws and regulations. Employees of the Sponsor, officers, directors, agents, employees, advertising and promotion agencies and members of their immediate families (spouses, parents, children, and siblings and their spouses) are not eligible.
4. How To Enter. No Purchase Necessary. To enter complete the official sweepstakes entry form online at www.milwaukeemoms.com. Alternate method of Entry: Mail a postcard to: Wizarding World Sweepstakes, c/o Milwaukee Moms, PO Box 661, 333 W. State Street, Milwaukee, WI, 53201. Include your Name, Address and daytime phone number. Only one sweepstakes entry per person permitted each day during the sweepstakes period. Multiple entries received from any person will void all such additional entries. Entries that are incomplete, illegible or corrupted are void and will not be accepted. Neither the Sponsor, the Milwaukee Journal Sentinel nor their respective parent companies, subsidiaries, affiliates, officers, directors, agents and employees are responsible for telecommunications, network, electronic, technical or computer failures, mail problems of any kind, for inaccurate transcription of entry information, errors in any promotional or marketing materials, errors in these rules, for any human or electronic error, or for entries that are stolen, misdirected, garbled, delayed by computer transmissions, lost, late or damaged. All entries become the property of the Sponsor and the Milwaukee Journal Sentinel and will not be acknowledged or returned. All alternate entries must be received by Wednesday, April 21, 2010 to be eligible for the drawing. Entrants agree that the Sponsors may contact the entrant via email or US Postal Mail, to provide you with information about goods or services. If you do not wish to receive these materials please indicate such on your mail in post card.
5. Prize.
a. Round trip, coach class air transportation for four (4) people from the major airport closest to winner’s home to Orlando, Florida provided by Southwest Airlines. Winner will be responsible for ground transportation to/from the airport at point of origin.
b. Non-exclusive ground transfers to/from the airport in Orlando, Florida and to/from the hotel and Universal Orland (UO).
c. Hotel accommodations, room and tax only, for up to four (4) people in a maximum of one (1) room at the Loew’s Portofino Bay Hotel® at Universal Orlando® (or another UO on-site hotel of UO’s choosing), for four (4) consecutive days and three (3) nights. Hotel accommodations do not include meals, incidentals, tips, telephone calls, or any other personal expenses incurred during the trip, not specified here.
d. Complimentary 3-Day 2-Park Tickets for four (4) people for admission to Universal Studios and Universal’s Islands of Adventure.
NOTE: The Prize must be must be completed no later than March 31, 2011. Blackout dates or other restrictions may apply as dictated by airline and/or hotel. Winner travel arrangements must be made a minimum of sixty (60) days prior to winner travel and are subject to availability. If winner is unable to fulfill prize during time period, winner forfeits the prize package. Winner must be 21 years of age or older. Minor traveling companion(s), if any, must be accompanied by a parent or legal guardian. Trips are non-transferable and have no cash value. Winner and guests must travel at the same time and must depart from the same city.
All the tickets are valid during normal operating hours only. Operating hours and availability of attractions and shows are subject to change without notice. Some special events may be separately ticketed. Universal reserves the right to change the name of all tickets. All tickets specifically exclude admission to special or separately ticketed “hard ticket” entertainment events at either of the theme parks, hotels, or within any of the Universal CityWalk venues such as Hard Rock Live® and the Blue Man Group show.
6. The prize cannot be exchanged for cash. Transfer of the prize is prohibited. Fair Market Value (FMV) of prize is $3,129.60 (actual value may vary depending on departure date and location). All taxes and all other expenses, costs or fees associated with the acceptance and/or use of the prize are the sole responsibility of the winner. The Sponsor reserves the right to substitute a similar prize of equal or greater value at its sole discretion. Winner must complete an affidavit of eligibility and liability release, or the prize will be forfeited and alternate winner selected. No substitutions are allowed.
7. Selection of Winner One (1) Grand Prize winner will be selected through a random drawing by sweepstakes officials at the Milwaukee Journal Sentinel. One winner will be drawn from the pool of all eligible entries on or about April 23, 2010. Winners will be contacted by phone at the number provided by the entrant. The odds of winning will depend on the number eligible entries received. Prize winners will be required to sign and return to the Sponsor an affidavit of eligibility and liability and publicity release and W9 tax form. If any prize notification is undeliverable, or in the event of non-compliance with any of these requirements, the prize will be forfeited, and an alternate winner will be selected at random from the remaining eligible entries.
8. Disclaimer. Winner assumes all liability for any injury or damages caused or claimed to be caused by his or her participation in the sweepstakes and/or the acceptance and/or use of any prize, and releases the Sponsor, the Milwaukee Journal Sentinel and their respective parent companies, subsidiaries, affiliates, officers, directors, agents, and employees, from any such liability. The Sponsor, the Milwaukee Journal Sentinel and their respective parent companies, subsidiaries, affiliates, officers, directors, agents and employees are not responsible for technical, hardware, software, or telephone malfunctions of any kind, lost or unavailable network connections, or failed incorrect, incomplete, inaccurate, garbled or delayed electronic communications caused by the user or by any of the equipment or programming associated with or utilized in this sweepstakes or by any human error which may occur in the processing of the entries in this sweepstakes. Entry constitutes permission (except where prohibited by law) to use winner's name, city, state, likeness and/or voice for purposes of advertising, promotion and publicity without additional compensation. The winner's name and city of residence will be posted online and mailed to those who request it.
9. Winner Announcement. For the names of the Prize winners, available after May 1, 2010, send a self-addressed, stamped envelope by or before December 1, 2010 to: Wizarding World Sweepstakes Winners c/o The Milwaukee Journal Sentinel, P.O. Box 661 Milwaukee, WI 53201-0661.
