Applying to be a contestant for the Fitness Transformation Competition:
No purchase is necessary to enter, and one valid entry is allowed per person.
You must be 18 years of age or older at time of entry to be eligible for this contest. This contest is void where prohibited by law.
To apply to be a contestant, you must first fill out the online application. After that application is reviewed by the staff of Fitness Together, you may be contacted to complete a more detailed application to determine whether you will be chosen as one of the six contestants.
If selected as a participant, you must execute waivers and release agreements required by Fitness Together. You will need to be available to Fitness Together for a five-month period. Any offer to become a participant is conditioned upon your submission to physical and psychological examinations to be conducted. You must be willing to submit medical information to Fitness Together.
Each of the six contestants will receive a free, five-month fitness training program provided by the six Milwaukee-area Fitness Together locations.
Throughout the competition, readers will be asked to vote for one of the six contestants. The grand prize winner of the competition will be the contestant who receives the most votes by Aug. xx, 2009. The decision of the judges is final.
Entering the drawing for the three-month fitness program, to be awarded in April, June and July:
The prizes will be awarded to registered users only. Each time a user votes for one of the contestants, he will be entered into a random drawing for prizes. The more votes you cast, the greater your chance of winning. The odds of winning are based on the number of entries received.
Voting is limited to one vote per day per email address.
Other rules:
All decisions by Fitness Together concerning selection of the contestants are not subject to appeal; and Fitness Together has no obligation to return any materials submitted by applicants as part of the application.
JSOnline reserves the right to restrict the contestants to only persons whose primary residence is within a 30-mile radius of a Fitness Together location.
Employees of Fitness Together, the Milwaukee Journal Sentinel, JSOnline, its respective parent, affiliates, subsidiaries, advertising sponsors and promotional agencies, and the immediate family members of each are not eligible.
Contest entrants agree to abide by the terms of these Official Rules and by the decisions of the contest commissioners, which are final on all matters pertaining to the contest.
Entrants further grant to JSOnline and Fitness Together the right to use and publish their proper name and state online and in print, or any other media, in connection with the Contest.
JSOnline reserves the right to use any and all information related to the Contest, including information on contestants obtained through the contest, for marketing purposes or any other purpose, unless prohibited by law.
The Contest and all of the related pages, contents and code are copyright, UPICKEM, Inc., and/or JSOnline. Copying or unauthorized use of any copyrighted materials, trademarks or any other intellectual property without the express written consent of its owner is strictly prohibited.
Each winner, by acceptance of the prize, agrees to release all Sponsors, and their parent and subsidiary companies, their officers, directors, employees, agents, shareholders, affiliates, suppliers, distributors, and advertising agencies from all liability, claims, or actions of any kind whatsoever for injuries, damages, or losses to persons and property which may be sustained in connection with the receipt, ownership, or use of the prize.
UPICKEM, Fitness Together and JSOnline are not responsible for Internet crashes or slowdowns caused by network congestion, viruses, sabotage, satellite failures, phone line failures, electrical outages, natural disasters or acts of man or God.
UPICKEM, Fitness Together and JSOnline shall not be liable for any injury or illness incurred by any contestants in this competition. Contestants will be required to sign a waiver that releases the sponsors from all liability
All results posted are unofficial until winners respond to e-mail notification.
Each winner is responsible for all federal, state and local taxes and will be required to complete an IRS form W-9. Sponsor reserves the right to withhold prizes until completed form W-9 is received.
JSOnline and Fitness Together reserve the right to change the rules at any time.
